Yes, if you want to go scuba diving, you will have to pay an additional 200 baht for a diving permit in Tarutao National Park, which includes Ko Lipe. That is, in total, you need to pay 200 baht as the standard fee for access to the national park (paid by everyone) + 200 baht for diving. The fee is one-time, not per dive. Children get a discount (100 baht each), local residents pay 40 baht. If you are just relaxing on Ko Lipe without diving, or snorkeling (swimming with a mask and snorkel), you do not need to pay this extra 200 baht.
