No, you don't need to fill out anything, at least not on paper. Now the entire procedure has been moved to electronic form, and all paper forms have been abolished.

The visa for Indonesia and Bali is issued electronically in advance or right here at the airport at special counters, again electronically. For this, all the work is done by the staff at the counter using your passport data, and you won't have to fill out anything; you'll only need to provide your email address where the electronic visa will be sent. Also, the customs declaration form BC2.2 is filled out electronically on the website https://ecd.beacukai.go.id/ or on specially installed computers at the airport, and you'll receive a QR code to present at customs control. And even in a new-style passport, nothing is stamped: the passport is simply scanned on scanners without interacting with border guards, and if the visa is paid for and issued successfully, you simply enter the country through an automatic turnstile.
